Supported Community Living Services

 

Are designed to give persons with disabilities an opportunity to live in their own apartment/home and/or waiver home which is integrated in the community. With support provided by Tenco staff, individuals are able to live semi-independently and in the least restrictive setting. With the support, individuals will hopefully be prevented from moving into a group living arrangement.

Through our program we are looking for each individual to maintain existing skills and acquire new skills enabling them to continue living in their own residential setting which is integrated in the community with minimal or no staff supervision.

 

 

Admissions Criteria:

  • Applicant must consent to admission.
  • Medical examination with 12 months prior admission.
  • Applicant must be able to meet their own personal needs with limited assistance from others.
  • Applicant must be able to communicate their needs.
  • The applicant shall meet the following conditions:
    1. Require minimal supervision, but not the level of care and supervision provided in licensed residential care facilities.
    2. Be diagnosed with a mental illness, intellectual disability, brain injury, or developmental disability.
    3. Be an adult.
  • Applicant must not be a serious danger to self or others.
  • Applicant has been sponsored for payment of fees or ability to private pay.
  • Approved and certified service status must be established before the receipt of HCBS/MR or HCBS/MR/OBRA waiver services funds.

Exit Criteria:

  • Individual outgrows need for support services.
  • Individual needs more support than can be provided in this program.
  • Individual lacks sponsorship for fees and/or becomes ineligible for services.
  • Applicant no longer meets eligibility criteria.
