Adult Day Habilitation

Center of Hope/Fulfillment Center

 

To provide individuals with disabilities an adult developmental program to develop, maintain, and increase/maximize their personal capabilities; physical and emotional growth, communication, socialization, community integration, and basic life skills.  Also to lend support to their interdependence with the outcome being choice and control in the quality of their lifestyle.

Individuals receiving services may be involved in individualized living skills, and recreation/leisure skills as determined by the individual’s goals and objectives.  Individuals in this program will have opportunities to participate in a variety of life experiences, which include skills training, rehabilitative services, and community participation through socialization, leisure, and cultural activities.  Activities may consist of swimming, bowling, movies, going out to eat, shopping and other age appropriate activities.

Staff to individual ratio, which includes Program Instructors, will be a maximum of 1.5:3.

 

 

 

 

 

Admissions Criteria:

  • Applicant consents to admission.
  • Applicant is 18 years of age or older.
  • Applicant has a disability.
  • Applicant is unable to meet his/her own personal needs.  Admissions Committee will review specific individual needs for possible procurement of additional services for individuals.
  • Applicant is not a serious danger to self or others.
  • Applicant is ambulatory or mobile non-ambulatory.
  • Applicant has been sponsored for payment of fees, or able to pay his/her own fees.
  • Applicant is unable to sustain work or other activities without constant supervision.

Exit Criteria:
  • Individual shows favorable prognosis for growth from other services.
  • Individual demonstrates basic independent social sensomotor and communication skills.
  • Individual can sustain non-work activities for duration of one (1) hour.
  • Written request to leave Day Habilitation Services.
  • Individual’s behaviors and/or cares are no longer appropriate for the Day Habilitation Services Program.
